Tax Exemption With Prescription Information

Orders for adaptive clothing with prescriptions are tax exempt! Give us a copy of your prescription, and you can save the tax on your order.

When placing your first order, tell us you have a prescription in the comments section. Once you have completed your order, do one of the following:

• Fax your prescription form and order confirmation number to us at 905-738-6236
• Scan your prescription form and email it to silvertscare@careismatic.com with your order confirmation number

Once we have your prescription form, we will keep it on file and remove the taxes from your order.
